Abstract: A multi-item producer-retailer integrated system featuring overtime and random scrap is studied. The objectives are to jointly decide the most economic rotation fabrication cycle time and distribution of products. In order to meet the increasing demands of diversified end items, production managers today need to plan a multiproduct fabrication schedule and to expedite both manufacturing and transportation times, so that they can meet product demands as quickly as possible. Also, due to potential uncontrollable reasons, scrap items are generated randomly in a real fabrication process. To address the aforementioned issues, this study examines a multi-item producer-retailer integrated system featuring overtime and random scrap. We build a mathematical model to interpret the proposed multi-item producer-retailer integrated system which incorporates shipping and retailer’s holding cost. The Hessian matrix equations are used for solving the optimality of the system. Diverse important system information can now be exposed to backing managerial decision makings, which includes individual and combined influences of variations in particular system factor(s) (such as scrap rate and overtime related parameters) on the specific system performance.

Abstract Organization environment has significant influence on occupational health among medical workers. In current study, based on previous established questionnaires, we constructed a new questionnaire to evaluate the effects of organizational environment on occupational injury of medical workers. In a large-scale of investigation, a total of 2350 medical workers from 54 hospitals in Zhuhai city, China, were surveyed by the new questionnaire, and 2111 (91.2%) questionnaires were used for final analysis. The average score of organization environment of medical workers questionnaire was 91.33 ± 21.95. We found that as compared with the average levels, medical workers have higher incidences of behavioral symptoms such as depressed mood, obsessional thinking, and anxiety and so on. We also demonstrated the questionnaire scores correlated with the incidence of occupational disease, occupational injuries and the scores of SCL-90, implying its potential application to objectively measure the conditions of medial safety of medical workers in China. In all, we concluded that organizational environment has important implications on the assessment of occupational health of medical workers.

Abstract: Auto mirrors are indispensable and essential for the reflection of objects behind the car and act as a crucial part in driving security. In manufacturing stages of auto mirrors, certain tasks operated unusually will cause scratches, chips, pinholes, bubbles and damaged edges on the general surface and profile faults on auto mirrors. Those appearance faults sometimes will severely have an impact on the standard of the mirror reflection and increase the driving hazard. During the traditional examination of auto mirrors in the manufacturing process, almost all work is performed by human examiners. Manual examination can be easily affected by foreign objects reflected on the mirror surfaces which can cause mistaken determinations of fault examination. Hence, this study works toward investigating the automatic appearance fault detection of auto mirrors. We propose a fault enhancement technique based on Fourier high-pass filtering and the convex hull arithmetic to inspect appearance faults on auto mirrors. This approach only utilizes their own information of testing images to judge whether there are any irregular appearance changes without the need of standard patterns for matching. Experimental outcomes illustrate that the appearance fault detection rate increases by 95.13%, the false alarm rate decreases to 1.88% and the correct classification rate attains to 98.11%.

Abstract A multiproduct fabrication-shipment problem with postponement and quality reassurance was explored (Chiu et al., 2016) to minimize overall relevant system costs, in which the differential calculus was utilized in the optimization procedure to help decide the optimal fabrication-distribution policy. This research note presents a two-phase algebraic approach, in lieu of their derivatives, to find the optimal policy to the problem. Such a simplified method aims at helping managers or practitioners in the field to understand and solve the problem more effectively, and without derivatives.

Abstract The roles that aortitis plays in the development of annuloaortic ectasia (AAE) remain uncertain, while clinical features of AAE in arteritis are largely unknown. This study was designed to highlight the clinical features of AAE, the treatments of choice, and the causative relations between aortitis and AAE. The morphology of the aortic valve leaflets was normal in half of the patients, while the valves were thin and overstretched in the other half. Most patients had an aortic aneurysm. Half of the patients had severe aortic valve insufficiency, and one-quarter of them had dilation of the sinuses of Valsalva. Takayasu arteritis was prone to develop coronary artery lesions, whereas giant cell arteritis were not. Aortic branch lesions in Takayasu arteritis were stenotic or occlusive in 92.9% of the patients, while in giant cell arteritis, they were all dilated lesions. Most patients (94.7%) required surgical treatment with steroid therapy. However, long-term follow-up results showed a higher anastomotic dehiscence rate, particularly in patients with Takayasu arteritis. Further morphometric and pathological research on AAE in arteritis should be undertaken, and more feasible measures should be warranted for preventing postoperative anastomotic dehiscence.

Abstract Postoperative cognitive dysfunction is a common complication following cardiac surgery. The incidence of cognitive dysfunction is more pronounced in patients receiving a cardiac operation than in those undergoing a non-cardiac operation. Clinical observations demonstrated that pulsatile flow was superior to nonpulsatile flow, and membrane oxygenator was superior to bubble oxygenator in terms of postoperative cognitive status. Nevertheless, cognitive assessments in patients receiving an on-pump and off-pump coronary artery bypass surgery have yielded inconsistent results. The exact mechanisms of postoperative cognitive dysfunction following coronary artery bypass grafting remain uncertain. The dual effects, neuroprotective and neurotoxic, of anesthetics should be thoroughly investigated. The diagnosis should be based on a comprehensive cognitive evaluation with neuropsychiatric tests, cerebral biomarker inspections, and electroencephalographic examination. The management strategies for cognitive dysfunction can be preventive or therapeutic. The preventive strategies of modifying surgical facilities and techniques can be effective for preventing the development of postoperative cognitive dysfunction. Investigational therapies may offer novel strategies of treatments. Anesthetic preconditioning might be helpful for the improvement of this dysfunction.

Abstract Objective: To demonstrate the underlying mechanisms of aortic dissection compared to those of coronary artery disease in terms of the transforming growth factor-beta (TGF-β) signaling pathway. Methods: Twenty consecutive aortic dissection patients and 20 consecutive coronary artery disease patients undergoing a surgical treatment in this hospital were enrolled into this study. The aortic tissues were sampled and the TGF-β1 and its receptor TGF-β receptor I (TβRI) were detected by Western blotting assay. Results: TGF-β1 and TβRI were positively expressed in the aortic tissues in both groups by Western blotting assay. The expressions of the two proteins were significantly higher in the aortic tissue of patients with aortic dissection than in those with coronary artery disease. The quantitative analyses of the relative gray scales of the proteins disclosed close correlations between the expressions of TGF-β1 and TβRI in both the study and control group patients. Conclusions: The aortic remodeling of aortic dissection might differ from that of coronary artery atherosclerosis concerning the nature, mechanism, mode, and activities of TGF-β signaling pathway. The development of aortic dissection could be associated with a significantly enhanced function of TGF-β1/Smad signaling transduction as a result of aortic remodeling incorporating both vascular injury and repair.

ABSTRACT The risk of transfusion-transmitted malaria is a major concern in many countries. This study investigated the prevalence of malaria antibodies and parasitemia in eligible blood donors in Jiangsu, in Eastern China. Malaria antibodies were detected in 2.13% of the 704 plasma samples studied. We found that the prevalence of malaria antibodies was not significantly correlated with gender, occupation and frequency of donation, but it increased with age. No Plasmodium was observed in red blood cells and no Plasmodium DNA was detected in any of the antibody-positive samples. The prevalence of malaria antibodies was not higher than expected in Eastern China.

Problem China continues to face challenges in eliminating mother-to-child transmission of human immunodeficiency virus (HIV), syphilis and hepatitis B virus (HBV). Approach In 2010, a programme that integrated and standardized prevention of mother-to-child transmission (PMTCT) efforts for HIV, syphilis and HBV was implemented in 1156 counties. At participating antenatal care clinics, pregnant women were offered all three tests concurrently and free of charge. Further interventions such as free treatment, prophylaxis and testing for mothers and their children were provided for HIV and syphilis. Local setting China’s national PMTCT HIV programme started in 2003, at which time there were no national programmes for perinatal syphilis and HBV. In 2009, the rate of maternal-to-child transmission of HIV was 8.1% (57/702). Reported congenital syphilis was 60.8 per 100 000 live births. HBV infection was 7.2% of the overall population infected. Relevant changes Between 2010 and 2013 the number of pregnant women attending antenatal care clinics with integrated PMTCT services increased from 5.5 million to 13.1 million. In 2013, 12.7 million pregnant women were tested for HIV, 12.6 million for syphilis and 12.7 million for HBV. Mother-to-child transmission of HIV fell to 6.7% in 2013. Data on syphilis transmission are not yet available. Lessons learnt Integrated PMTCT services proved to be feasible and effective, and they are now part of the routine maternal and child health services provided to infected women. The services are provided through a collaboration between maternal and child health clinics, the national and local Centers for Disease Control and Prevention, and general hospitals.

Resumen Situación China sigue haciendo frente a desafíos en la eliminación de la transmisión maternoinfantil del virus de la inmunodeficiencia humana (VIH), la sífilis y el virus de la hepatitis B (VHB). Enfoque En 2010 se puso en marcha en 1156 condados un programa que integraba y estandarizaba los esfuerzos de prevención de la transmisión maternoinfantil (PTMI) para el VIH, la sífilis y el VHB. En las clínicas de atención prenatal que participaron se ofrecieron a las mujeres embarazadas las tres pruebas al mismo tiempo y de forma gratuita. El programa incluyó otras intervenciones como el tratamiento gratuito, profilaxis y pruebas para las madres y sus hijos. Marco regional El programa nacional de PTMI del VIH en China comenzó en 2003, momento en el que no había ningún programa nacional para la sífilis y el VHB perinatales. OBJECTIVE: To estimate influenza-associated mortality in urban China. METHODS: Influenza-associated excess mortality for the period 2003-2008 was estimated in three cities in temperate northern China and five cities in the subtropical south of the country. The estimates were derived from models based on negative binomial regressions, vital statistics and the results of weekly influenza virus surveillance. FINDINGS: Annual influenza-associated excess mortality, for all causes, was 18.0 (range: 10.9-32.7) deaths per 100000 population in the northern cities and 11.3 (range: 7.3-17.8) deaths per 100000 in the southern cities. Excess mortality for respiratory and circulatory disease was 12.4 (range: 7.4-22.2) and 8.8 (range: 5.5-13.6) deaths per 100000 people in the northern and southern cities, respectively. Most (86%) deaths occurred among people aged >65 years. Influenza-associated excess mortality was higher in B-virus-dominant seasons than in seasons when A(H3N2) or A(H1N1) predominated, and more than half of all influenza-associated mortality was associated with influenza B virus. CONCLUSION: Between 2003 and 2008, seasonal influenza, particularly that caused by the influenza B virus, was associated with substantial mortality in three cities in the temperate north of China and five cities in the subtropical south of the country.

OBJETIVO: Calcular la mortalidad asociada a la gripe en la China urbana. MÉTODOS: Se calculó el exceso de mortalidad asociado a la gripe durante el periodo comprendido entre 2003 y 2008 en tres ciudades del norte de China con clima templado y en cinco ciudades del sur del país con clima subtropical. Los cálculos se obtuvieron de modelos basados en regresiones binomiales negativas, estadísticas vitales y de los resultados de la vigilancia semanal del virus de la gripe. RESULTADOS: El exceso de mortalidad anual asociado a la gripe, por todas las causas, fue de 18,0 (rango: 10,9-32,7) muertes por cada 100000 habitantes en las ciudades del norte y de 11,3 (rango: 7,3-17,8) muertes por cada 100000 habitantes en las ciudades del sur. La mayor parte de este exceso de mortalidad - 12,4 (rango: 7,4-22,2) y 8,8 (rango: 5,5-13,6) muertes por cada 100000 habitantes en las ciudades del norte y del sur, respectivamente - se atribuyeron a una enfermedad respiratoria y/o circulatoria. La mayoría de las muertes (el 86%) ocurrió en personas con una edad >65 años. El exceso de mortalidad asociado a la gripe fue superior en épocas con un virus B dominante que en épocas en las que predominaron los virus A(H3N2) o A(H1N1). La plus grande partie de cette mortalité excessive - respectivement 12,4 (plage: 7,4-22,2) et 8,8 (plage: 5,5-13,6) décès pour une population de 100 000 personnes dans les villes du nord et du sud - a été attribuée à des maladies respiratoires et/ou circulatoires. La plupart des décès (86%) sont survenus chez des personnes de >65 ans. La mortalité excessive associée à la grippe a été plus élevée lors des saisons où prédominait le virus B plutôt que lors de celles où prédominaient les virus A(H3N2) ou A(H1N1) et plus de la moitié de l'ensemble de la mortalité associée à la grippe a été associée au virus B de la grippe. CONCLUSION: De 2003 à 2008, la grippe saisonnière, surtout celle provoquée par le virus B, a été associée à une mortalité substantielle dans trois villes du nord tempéré de Chine et dans cinq villes du sud subtropical du pays.
